Membership of Unique is free for families and individuals affected by rare chromosome disorders but donations are very gratefully received to help us continue our work. For membership please give us:

• your full name
• full postal address with post code or zip code and the country you live in
• email address
• telephone number with area code (and country code if you live outside the United Kingdom)

Please make sure that you give these details to us accurately so that we can get back to you. Sometimes different internet service providers are incompatible and your email messages to us will be bounced back to you. If this happens, please try again and if that does not solve the problem, please telephone, fax or write to us instead

Please also give us details of your own and/or your child's rare chromosome disorder, including the karyotype with any breakpoint numbers or the results of any analyses such as FISH or array CGH If you do not know your own or your child's karyotype,FISH or array CGH results then you can ask your doctor or geneticist. If possible, please also let us have copies of the original lab reports explaining the results. If you are not sure what a karyotype, FISH or array CGH are then please read our Little Yellow Book for an explanation
